Test whether your risk controls work in practice.
Beaumont Ridge provides independent, evidence-led assurance reviews for health and social care organisations responsible for the quality and safety of care.
We examine whether risks are identified, assessed, controlled, monitored, escalated, learned from and reduced, and whether documented assurance is supported by practice and outcomes.
Assurance beyond paperwork.
We do not simply ask whether a risk assessment has been completed. We examine whether the organisation's approach to risk actually works in practice.
Our reviews compare organisational statements with documentary evidence, stakeholder experience, frontline practice, the care environment and available outcomes.
The aim is to provide an independent and proportionate view of whether controls are appropriately designed, consistently implemented and effective for the people, setting and risks concerned.
Choose the assurance question you need answered.
Each review stands alone, and they can be combined where the assurance question spans governance, safety and environment.
Governance Assurance Review
Assess whether governance systems provide reliable oversight of quality and safety, identify and mitigate risk, maintain accurate records, act on feedback and drive sustained improvement.
Typical reasons to commission- Preparing for CQC or commissioner scrutiny
- Repeated incidents, complaints or safeguarding concerns
- Weak audit or risk assurance
- Leadership or organisational change
- Post-enforcement or improvement assurance
- Strengthening board or registered-manager oversight
Patient Safety Risk Review
Assess whether the organisation has an effective system for identifying, understanding, controlling, monitoring and learning from patient-safety risks. This is broader than one risk assessment or a single incident investigation.
Typical reasons to commission- Recurring incidents or near misses
- Safety concerns across a ward, service or pathway
- Post-incident assurance
- Pre-opening or post-change review
- Board or commissioner concern
- Weak evidence that safety actions are reducing risk
Ligature & Environmental Safety Review
Assess environmental hazards in context, including the interaction between the physical environment, people, care needs, observation, staffing, equipment, maintenance, operational practice and governance. This is not simply a ligature-point count.
Typical reasons to commission- Mental health, learning disability or complex-care environments
- New builds, refurbishments or acquisitions
- Post-incident review
- Emerging environmental concern
- Annual or thematic assurance
- Commissioner due diligence
- Pre-opening assurance
A stronger test of how risk is managed.
Assurance beyond paperwork
We test policy against records, practice, experience, environment and outcomes.
Risk in context
We consider how people, workforce, environment, care systems, leadership and organisational culture interact.
Proportionate professional judgement
Findings distinguish immediate concern, significant control weakness, improvement need, development opportunity and positive assurance.
Actionable recommendations
Recommendations identify the intended outcome, priority, suggested owner, evidence of completion and measures of effectiveness.
Re-assurance
Where commissioned, follow-up work tests whether action has changed practice and reduced residual risk, not simply whether a task was marked complete.
Define. Examine. Triangulate. Improve.
Every review follows the same eight-stage discipline, scaled to the agreed scope and assurance question.
Define
Agree the assurance question, scope, settings, population, exclusions, stakeholders, evidence period and reporting route.
Evidence
Review relevant policies, records, datasets, incidents, complaints, safeguarding intelligence, audits, risk registers and action plans.
Assess
Examine selected cases, systems, environments and controls using a structured review framework.
Engage
Speak with people using services, representatives where appropriate, frontline staff, specialists, managers and leaders.
Observe
Observe environments, routines and practice where proportionate, safe and ethically appropriate.
Triangulate
Compare organisational statements, documents, stakeholder accounts, observations and available outcomes.
Assure
Assess the strength of evidence and controls, escalate urgent issues, and identify systemic themes and positive practice.
Improve and re-assure
Set prioritised actions and, where commissioned, return to test implementation, effectiveness and residual risk.
Conclusions supported by proportionate evidence.
Sufficiency
Enough evidence to support the conclusion.
Reliability
Preference for contemporaneous, attributable and corroborated evidence.
Triangulation
Material conclusions should not depend unnecessarily on one source.
Proportionality
Depth reflects the risk, scope and commissioning question.
Fairness
Factual correction is distinguished from disagreement with professional judgement.
Transparency
Limitations, exclusions and evidence gaps are stated.
Governance Assurance Review
The Governance Assurance Review examines whether governance systems are established, understood and operated effectively in practice.
It tests whether leaders receive reliable information, risks and quality concerns are identified and escalated, records support safe decision-making, feedback informs improvement, and actions are completed and evaluated.
The review can be aligned with Regulation 17, the CQC assessment framework and other agreed regulatory or commissioning requirements.
Leadership and accountability
Governance structure, delegated authority, role clarity, escalation routes, challenge and oversight.
Quality assurance and audit
Audit design, coverage, sampling, findings, action tracking and whether assurance activity improves care.
Risk management
Risk identification, scoring, controls, ownership, escalation, review and residual risk.
Records and information
Accuracy, completeness, contemporaneous recording, data quality, version control and decision trails.
Feedback and engagement
How feedback from people, representatives, staff, commissioners and partners is gathered and converted into action.
Improvement and sustainability
Learning from incidents, complaints and safeguarding; accountable action; effectiveness measures and sustained improvement.
